Welcome to Homestead Girls Lacrosse! 
There are a few steps that need to be completed in order to participate. 
 
1. Please read this document: General Lacrosse Information
2. Register with USALacrosse ($35 fee)
3. Fill out: Athlete Registration Form (you will need USA Lacrosse Membership Number)
4. Fill out: Medical Information Form
5. Download, Fill out, Email these forms to [email protected] OR hand completed forms to Coach Emily
Release and Liability   Sudden Cardiac Arrest    Consent to Treat
6. Payment options:
    a. Send payment $500 VIA Venmo @HomesteadGirlsLax2025
    b. Check made out to Fort Wayne Lacrosse Association, Inc, hand to a board member or coach
 7.Register here: https://www.indianagirlslacrosse.com (you will need USA Lacrosse Membership Number)

Forms should be submitted before your daughter begins practicing.
